Renewal notices are sent out to the individual's home address approximately 45-50 days in advance of the expiration date. Please keep the IDPH informed of changes of address. The renewal fee and return address are noted on the renewal notice. The permit is mailed to the place of employment unless otherwise requested.
The permit expires on the date shown on the permit. There is no grace period for permit renewal. Renewals should be received in the bureau before the 15th of the month prior to the expiration date or the renewal process may not be completed by the expiration date. The individual may not be allowed to work after the permit expires even though the paperwork is being processed.
Submit with the renewal notice all CE proofs of attendance completed that are not listed on the computer compilation record included with the renewal notice. Failure to provide copies of proofs may delay the issuance of your permit before the expiration date.
If an extension is needed to earn continuing education, requests for the extension plus the $25 request fee must be received in this bureau by the expiration date or the permit is considered expired. If it is determined after the expiration date that an extension is needed, the extension will be granted upon receipt of the $25 request fee. A letter granting the extension is issued for 60 days from the expiration date of the permit.
Facilities are notified of expired permit holders approximately 30 days after the expiration date. A copy is sent to the permit holder.
