The Community Transformation Grant (CTG) is a program of the Center for Disease Control and Prevention (CDC). The grant's purpose is to reduce risk factors responsible for the leading causes of death and disability by creating healthier communities.
Over the next five years, 26 counties in Iowa will work on Iowa's Community Transformation Grant. They will focus on four main areas, also known as strategic directions. These areas include: Tobacco Free Living, Active Living & Health Eating, Clinical Preventive Services, and Safe & Healthy Environments.
